Calian Military Family Doctor Network
Calian Health, in partnership with the Department of National Defence (DND), is pleased to present the Calian Military Family Doctor Network. The goal of the program is to improve access to quality healthcare for the spouses, children and dependents of serving members of the Canadian Armed Forces.
The program has been launched where the need is great and there are a number of family physicians willing to extend their practices to accommodate military families. This network will help to expedite the process of obtaining a physician for military families when they are relocated and significantly reduce the number of military families without a physician.
Over time, access to the network of over 400 family physicians at over 145 Primacy Clinics across Canada will be available to military families. More program information.
How do I sign up for the program?
- Simply obtain and complete a doctor network form by visiting your local Military Family Resource Centre or by downloading the PDF version specific to your area (below).
- Once the form is complete, submit it to Calian’s Primacy team by faxing it to (888) 511-0219, or scan and email it to firstname.lastname@example.org
- A representative from Primacy will contact you to confirm receipt and ask any additional questions necessary.

What should I expect?
A representative from Calian’s Primacy team will be in touch to review your requirements and work with you to help you find a family physician. If you have any questions, call the Primacy team at 1-877-633-7722 x550.
Who is Eligible for the Program?
Serving Canadian Armed Forces family members who are not eligible for medical services from the Department of National Defence are encouraged to apply. Unfortunately, we cannot accept veterans or members releasing for medical reasons at this time. Please stay tuned for updates.
When can I sign up?
As soon as you know that you will be posted to a base with a participating Resource Centre and will be looking for a family doctor. It is best to get the process started early.
