If you have a form to be filled in, please tell the receptionist when checking in and we will try to assist you.
There are fees for certain forms.

Prescriptions

We do not renew prescriptions or give test results over the phone, however, we will try to answer your calls personally.
Please note: our physicians do not routinely carry triplicate prescription pads.
Make sure you renew your prescription at least a week before you run out of medication.
